Terms of Service

1. Private service; no government affiliation

CHS is a private business. We are not the Government of Canada, the Government of Alberta, Alberta Health, AHCIP, ServiceOntario, Health Insurance BC, or a registry agent. We are not affiliated with, endorsed by, sponsored by, or authorized by those organizations.

The service helps you enter information into an official government form and produces a downloadable PDF for you to review, print, sign where required, and submit yourself. We do not submit applications, deliver documents to a registry, issue health cards, determine eligibility, provide coverage, or guarantee approval or processing times.

2. What you purchase

You purchase a one-time private form-preparation service at the price displayed before payment. The payment is a CHS service fee and is not a government fee. There is no subscription unless the checkout page expressly says otherwise.

The service is based on the information you provide. You are responsible for reviewing every field and the generated PDF before signing or submitting it. You must correct errors and provide any originals, translations, supporting records, or additional information required by the relevant government office.

3. Your responsibilities

• Provide truthful, complete, and current information.
• Only provide information and signatures you are authorized to provide. If completing a form for another person, you confirm that you have the necessary authority.
• Keep your browser/device secure and download or print your completed PDF promptly.
• Follow the applicable government instructions and submit the application through the required official process.

CHS does not provide legal, medical, immigration, tax, or government-advice services. Ask the relevant government office or a qualified professional if you are unsure what a form requires.

4. Payment, delivery, and refunds

Payment is processed by a third-party payment provider shown at checkout. After payment is verified, the completed PDF is made available through the site. Card details are handled by the payment provider and are not entered into the CHS form flow.

Because this digital service is delivered immediately and access to the completed PDF is unlocked after checkout, payments are generally non-refundable once the service has been delivered. This policy does not limit any cancellation, refund, remedy, or other right that cannot lawfully be excluded or waived. If you were charged in error or the service failed to deliver the purchased access, contact us promptly so we can investigate and provide any remedy required by law.

5. Intellectual property and acceptable use

CHS owns or licenses the site’s software, branding, and original content. You receive a personal, non-exclusive right to use your completed PDF for your own application. Do not resell, republish, reverse engineer, interfere with the site, or use it to submit information you are not authorized to submit.

6. Disclaimers and limits

The site and service are provided on an “as available” basis. We do not promise that a government form, website, office, eligibility rule, or process will remain unchanged; that the generated PDF will be accepted; or that an application will be approved, processed by a particular date, or result in a health card.

To the maximum extent permitted by applicable law, CHS is not responsible for government decisions, delays, lost originals, inaccurate information supplied by you, interruptions outside our reasonable control, or your failure to review and submit the form. Nothing in these Terms excludes liability or consumer rights that cannot legally be excluded or limited.

7. Changes and contact

We may update these Terms when the service or law changes. The effective date above shows when this version took effect. Continued use after an update means you accept the updated Terms.
